Care Management Social Worker

Mercy is seeking a PRN/As Needed Care Manager to coordinate patient care across the continuum, ensuring safe transitions, appropriate resource use, and strong patient advocacy. In this flexible, as-needed role, you will conduct clinical and psychosocial assessments, develop individualized care plans, and collaborate with physicians, nurses, social workers, and community partners to optimize outcomes. You will educate patients and families, arrange post-acute services, address barriers to care, and support complex discharge planning while documenting thoroughly in the EMR and upholding Mercy’s compassionate, patient-centered values.
