Social Worker II Job Summary: Under the supervision of a Mental Health Leadership Team Member, the Mental Health Provider, Access, will provide mental health services in an integrated health care agency serving UW-Madison students across identities and programs. To this end, the Mental Health Provider performs the following: DIRECT SERVICE (50-60%):
1. Primary responsibility is to provide access appointments over the phone and in person for students seeking services. This includes risk
assessment, service evaluation, and collaborative decision making. At times, brief interventions may also be used;
2. Other responsibilities may include:
1. Maintains accurate and timely documentation for all clinical interactions and consultations
2. Serve as needed on multidisciplinary teams within UHS and on the campus.
3. Involved in the development and implementation of programs and services related to access/triage and care management for students.
4. Collaborates with providers and staff in UHS units and with campus partners;
5. Follows agency policies and procedures consistent with state mental health statutes, ethical guidelines, best practices, HIPAA, and
FERPA;
6. Attends staff, and workgroup meetings;
7. Attends Mental Health sponsored professional development activities;
8. Other duties as assigned This vacancy is being announced simultaneously as Psychologist, Counselor and Social Worker; please apply under the appropriate credential for consideration. - Facilitate the referral of students from UHS to mental health care and resources in the community, as clinically indicated;
- Provide care management for students including for those involved in other UHS services: psychiatry, behavioral health, and counseling;
- Consult with UHS staff regarding resources and community referrals;
- Assist students in understanding their insurance benefits or connecting them to insurance resources;
- Assist students in obtaining access to resources including food, housing, and financial supports;
- Provide follow up, support, and advocacy as needed to students who have been referred to the community; and
- Provide other services such as individual, couples, and/or group therapy; on-call; or other clinical services to meet agency need.

- May provide expertise, training, and guidance to the community, peers, and/or students
- Recommends appropriate individual and family treatment plans according to departmental guidelines and standards
- Updates case records in accordance with agency and departmental guidelines and policies
- Facilitates interventions according to treatment plans and organizational policies
- Facilitates individual and group counseling resources and programs
- Provides crisis intervention as needed
- Provides referrals to establish coordinated treatment with community and state agencies
- Evaluates and assesses difficulties in psychosocial functioning in clients
- Develops and implements specialized intervention plans to support the needs of clients
- Performs social, emotional, and environmental evaluations
- Provides psychotherapy services to clients under general supervision
- Participates in treatment programs alongside an interdisciplinary team to diagnose or enhance social functioning of clients

- 1 or more years in a mental health setting
- LCSW - State Licensure required. Must be independently licensed in the State of Wisconsin. Or if APSW, must complete hours requirements for licensure within 1 year of hire. Must complete all licensure requirements for independent practice within 15 months of hire, with verification of submission of all licensure materials to DSPS. If you are an APSW, you must have completed 1000 qualifying hours.
- 2+ years in a mental health setting
- Demonstrated ability to provide brief assessment, care management, risk assessment/safety planning, and advocacy for clients
- Experience identifying resources within in-network and out-of-network insurance plans;
- Demonstrated skill in working well under time pressure to prioritize and manage multiple tasks
- Experience as a collaborator on a multidisciplinary team;
- Administrative and technological skills required to effectively document in an Electronic Health Record
