Professional development through the Clinicians in Action program. Responsibilities Primarily provide outpatient rehabilitation services in an in‑home setting for the older adult population. Evaluate and treat in the most functional environment – the patient’s home or a private residence or independent living facility. Assess, develop, and deliver physical therapy programs to facilitate rehabilitation. Make referrals to other disciplines to address comprehensive patient needs. Implement individualized therapy programs designed to restore, reinforce, and enhance performance. Instruct, educate, and train patients and caregivers in essential skills and functions to promote independence and productivity. Promote activities that maintain health and prevent pathology. Provide a flexible schedule and work environment in various settings. Accountabilities and Additional Duties Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements. Adhere to department and patient treatment schedule, timely clinical and billing documentation, and work‑related meetings. Attend and contribute to patient care, staffing conferences, and other related meetings. Consult with rehabilitation team to select appropriate therapeutic regimens. Adapt program and/or plan of care according to individual patient needs. Promote a safe environment and enforce removal of fire and safety hazards. Order supplies and equipment as necessary; maintain, identify, and report equipment needs and repairs. Provide patient‑centered service ensuring high‑quality compassionate care. Screen, examine, and evaluate patients; perform history, systems review, and appropriate tests. Interpret findings to establish diagnosis, identify impairments, and predict improvement. Develop treatment goals and methods in collaboration with patients and caregivers. Implement the physical therapy treatment plan and complete all documentation. Utilize therapeutic techniques – including exercise, manual therapy, airway clearance, electrotherapy, modalities, health education, falls assessment, and case management. Design, fabricate, fit, and train patients on adaptive devices, orthotics, and prosthetics. Instruct patients in skills and techniques for mobility activities. Perform routine reexamination to modify or progress treatment plan. Incorporate health and wellness programming into treatment plans. Perform home and community mobility assessments and provide safe mobility recommendations. Perform other related duties as required. Qualifications Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility. Master’s degree in Physical Therapy, or Master’s in a related field with a Bachelor’s in Physical Therapy, or Bachelor’s in Physical Therapy with 5 years of progressively responsible PT experience. For degrees earned outside the U.S., U.S. equivalent evaluation required. Valid driver’s license or reliable, independent transportation to patients’ residences.
