Key Responsibilities
- Evaluate and treat patients in a Skilled Nursing Facility (SNF) setting
- Develop and implement individualized physical therapy treatment plans
- Monitor and document patient progress toward established therapy goals
- Collaborate with interdisciplinary care teams to support patient recovery and functional outcomes
- Educate patients and caregivers on therapeutic exercises and home programs
- Active Physical Therapist (PT) license in the state of Oregon or eligibility to obtain
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
- Experience working in a Skilled Nursing Facility (SNF) setting
- Prior travel or contract therapy experience
- Familiarity with SNF documentation and Medicare compliance requirements
- Opportunity to gain valuable SNF experience through a 13-week contract placement
