Seven Healthcare are seeking a skilled Registered Nurse Case Manager to join a dedicated team at URMHC. This travel assignment requires expertise in home health care and is ideal for someone with certified experience in this field. The work environment is dynamic, supporting the needs of patients in various settings, with a core focus on direct patient interaction.

- Develop and revise plans of treatment and discharge plans in conjunction with patients, families, physicians, care teams, and payers.
- Complete and submit OASIS assessments, reassessments, transfers, resumptions, and discharges.
- Process orders, notify physicians of patient needs, and complete discharge summaries.
- Provide an environment respecting patient privacy, confidentiality, and property.
- Initiate preventive and rehabilitative strategies and provide services requiring specialized nursing skills.
- Consult with and educate patients, families, and team members regarding disease processes and strategies.
- Supervise LPNs to ensure patient health and compliance with care plans.
- Direct, coordinate, and evaluate the quality of care services provided by the Home Health Agency.
- Implement Care Management philosophy to promote quality and cost-effective outcomes.
- Complete clinical records for compliance with legislation and service delivery timeframes.
- Maintain current knowledge of available services and demonstrate flexibility in patient visits.
- Ensure problem resolution at the point of contact and attend patient care conferences.
- RN License (NY License ONLY)
- One year of experience as a clinical nurse in a hospital and nine months of experience in home care preferred
- Certified Home Health Care Experience Required
- EPIC or Netsmart Preferred
- OASIS Experience Required
- Telemetry Experience Required
- Strong critical thinking and communication skills, with the ability to make clinical judgments
- BLS certification (any provider accepted)
- Current NYS driver's license
- Ability to work independently and manage a variety of assignments
