Clinical Assessment Manager (UAS Required)

Locations: Queens, Brooklyn, or Brooklyn/Lower Manhattan, NY
Orientation: Bronx, NY
Setting: Field-Based | In-Home Participant Assessments
Schedule: Monday–Friday | 8:30 AM–5:30 PM (1-hour lunch)
Position Type: Full-Time | Temporary Assignment with Potential for Permanent Hire

About the Opportunity

A leading healthcare organization is seeking experienced Clinical Assessment Managers (RNs) to conduct comprehensive clinical assessments for participants throughout New York City.

Current opportunities are available in the following territories:

• Queens

• Brooklyn

• Brooklyn/Lower Manhattan

All positions require orientation in the Bronx.

This is a field-based role focused on completing in-home participant assessments, coordinating care, and ensuring compliance with New York State assessment requirements.

The ideal candidate is an experienced RN with strong hands-on UAS-NY experience who is comfortable working independently in the field and managing a designated territory.

Must Haves

• Active New York State Registered Nurse (RN) license

• Hands-on UAS-NY experience — REQUIRED

• Experience completing initial, annual, and significant change UAS assessments

• Strong comprehensive clinical assessment and care-planning skills

• Experience conducting in-home participant assessments

• Medication reconciliation and medication review experience

• Experience performing post-hospital discharge assessments

• Excellent clinical documentation and critical-thinking skills

• Ability to travel throughout the assigned territory

• Valid driver's license and reliable transportation, as required for the assigned territory

Preferred Experience

• Managed Long-Term Care ( MLTC )

• PACE

• Home Care

• RN Care Management or Case Management

• Medicare and Medicaid populations

• Geriatric or medically complex patients

• Bilingual skills are a plus

Responsibilities

• Complete UAS-NY assessments for new enrollments, annual reassessments, and significant changes in condition

• Conduct comprehensive in-home clinical assessments

• Perform medication reconciliation and medication reviews

• Complete post-hospital discharge evaluations

• Identify participant needs and communicate findings to the interdisciplinary care team

• Ensure accurate, timely, and compliant clinical documentation

• Coordinate with providers, caregivers, and community resources

• Manage an assigned geographic territory efficiently

• Maintain productivity and quality standards while delivering participant-centered care
