Communicates efficiently with supervisors and physicians about changes in patients condition. Is able to respond quickly and accurately to changes in condition or response to treatment./ppEssential Functions:/pulliPerforms initial and on-going physical and psycho-social assessment according to accepted standards of nursing practice./liliAssesses plans and evaluates patient care needs./liliCarries out physician orders./liliAdministers prescribed medications, changes dressings, cleans wounds, and monitors vital signs./liliParticipates in treatment team conferences to assist in planning and revising goals, objectives and interventions appropriate to the age-related and problem-specific needs of each patient./liliImplements nursing plan of care for assigned patients and conducts and/or co-leads group therapy sessions for patients./liliEvaluates patients response to interventions and revises nursing plan of care as needed./liliCollaborates with the treatment team to revise goals, objectives and interventions appropriate to the changes in patient status./liliMonitors, records and communicates patient condition as appropriate utilizing computerized documentation systems./liliInstructs and educates patients and families./liliProvides age and culturally appropriate care./liliFollows Standard Precautions using personal protective equipment as required./li/ulpQualifications:/pulliFL LPN License/liliCurrent BLS Certification as a Healthcare Provider (American Heart Association)./liliPrevious work experience in a long term healthcare setting preferred./li/ul/div
