PRN Hospice & Palliative NP - In-Home Care Specialist

Constellation Health Services in Cincinnati, OH is seeking a Nurse Practitioner focused on Hospice/Palliative Care for a PRN/per diem role. DEA license is required and candidate must hold Ohio nursing licensure.
You will evaluate patients, perform face-to-face visits, develop care plans with the hospice team, and educate families. Prior hospice/home health experience is preferred; CPR certification required.
