Social Services Director

CareCore at Westmoreland is a skilled nursing and rehabilitation facility in the Chillicothe area is seeking an experienced and compassionate Social Services Director to join our team. This position plays a vital role in supporting residents and families through admissions, discharge planning, care coordination, emotional support, and advocacy throughout their stay.
