Job Summary The Spiritual Care Coordinator (SCC) is responsible for fostering a culture of compassionate care by providing emotional and spiritual support to hospice patients and their caregivers. This includes coordinating with community faith leaders, facilitating religious practices, and, when appropriate, participating in the admission process alongside another clinical team member. In addition to direct patient care, SCCs play a vital role in supporting the well‑being of Minnesota Hospice’s clinical and professional staff. Their contributions may include leading memorial services, guiding meditations during staff meetings, facilitating spirituality groups, and providing opportunities for debriefing. As spiritual care leaders, SCCs may also develop and facilitate wellness programs designed to nurture staff resilience and self‑care. These activities can include in‑service education, role‑modeling healthy practices, and encouraging empowerment and self‑care—key components of the Minnesota Hospice mission. Supervisory Responsibilities None Duties/Responsibilities Provide emotional support, spiritual care and religious practice within a person’s unique context Posses psychological, sociological, cultural and interfaith competencies Participate in all interdisciplinary team meetings, staff meetings and educational in‑services Maintain communication with facility and home caregivers through the use of hospice binder calendars and visit record note entries at each visit Serve as a liaison between members of the clergy and the patient to support the patient and patient’s caregiver’s unique spiritual care needs and beliefs Adhere to all CMS standards regarding Spiritual Care assessments and charting. Coordinate and communicate patient changes to appropriate RN Case Manager Conduct self in a safe and professional manner during the performance of all job related duties Ability to be on call on weekends or evenings when patient family needs require assistance due to a patients transitional status Ability to assist in the admissions process Build and maintain partnerships with communities we serve, clergy, faith based groups and care organizations to support the spiritual care needs of the patients and families Required Skills/Abilities Knowledge of appropriate HIPAA and confidentiality Keen eye for attention to detail Able to work as both a part of a team and independently Possesses great organizational skills Posses a valid drivers license Education and Experience Masters of Divinity or equivalent from accredited seminary, required Three units of Clinical Pastoral Education, required; must be willing to complete four total units Ecclesiastical standing and/or board certification eligible Company Expectations Adhere to all Minnesota Hospice policies and procedures Promote teamwork Lead by example Conduct yourself in a positive, confident and professional manner Communicate clearly, concisely and accurately Promote the hospice concept within the communities we serve Offer a willingness to participate in Minnesota Hospice team building and community events Posses a valid driver’s license and auto insurance coverage This job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employer.
