Description

• Conduct comprehensive evaluations for patients in inpatient or outpatient settings and create personalized treatment plans
• Implement physical therapy interventions that focus on enhancing mobility, strength, balance, and functional capabilities
• Collaborate with a multidisciplinary team, including nurses, physicians, case managers, and other rehabilitation professionals to achieve optimal outcomes
• Educate patients and their families on therapeutic milestones, discharge preparations, and recommended home exercises
• Maintain detailed documentation of evaluations, treatments, and patient progress in line with all facility and regulatory guidelines
• Engage in care team rounds and cross-disciplinary meetings to ensure integrated care delivery

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

• State license to practice as a Physical Therapist
• CPR certification strongly preferred
• Additional certifications (e.g., geriatric, neurological) appreciated but not mandatory
