Senior Case Manager — Community Living & Crisis Support

ICL Bronx is seeking a Senior Case Manager to support consumers living in independent apartments within the community. You will help clients access medical, social, psychosocial, educational, financial and vocational services to promote independent living. Responsibilities include counseling, developing and reviewing treatment plans, maintaining records, and coordinating with providers.
