Senior Case Manager: Independent Living & Recovery Support

ACMH, INC. in Manhattan seeks a Senior Case Manager/Case Manager for the Manhattan Treatment Apartment program. You will provide case coordination and deliver community rehabilitation services, reporting to the Program Director. The role involves home visits, psychosocial histories, liaison with psychiatric care, access to services, medication monitoring, and development of individualized Service/Support Plans.
