- Evaluate and assess patients in a Skilled Nursing Facility (SNF) setting to develop individualized treatment plans
- Deliver evidence-based physical therapy interventions to improve patient mobility, strength, and functional independence
- Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory requirements
- Collaborate with interdisciplinary care teams to coordinate comprehensive patient care
- Educate patients and caregivers on home exercise programs and injury prevention strategies
- Graduate of an accredited Physical Therapy program
- Current and valid Physical Therapist (PT) licensure in the state of Illinois
- Experience working in a Skilled Nursing Facility (SNF) setting
- Current CPR/BLS certification
- Prior travel or contract therapy experience
- Familiarity with SNF documentation and compliance standards
- Opportunity to gain diverse clinical experience in a SNF environment through a 13-week contract placement
