Job Summary: Lewisburg / Williamsport area The Renal Dietitian is an important part of the Interdisciplinary Team delivering quality care to ESRD patients. Assessment, education, documentation and setting goals to achieve optimal patient outcomes and improve quality of life are responsibilities of this position.

Principle Duties and Responsibilities:

• Documentation of nutrition assessment, plan of care, instruction, and development of goals as defined under Conditions of Coverage.
• Participates as an interdisciplinary team member in developing long and short-term care plans as defined under Conditions of Coverage.
• Patient, family, and staff education
• Assists in the development of the dialysis prescription based on Kinetic modeling and established protocols.
• Completes Bone Management responsibilities following protocol guidelines and assist nurses with appropriate medication dosing.
• Participates in monthly QAPI meetings reporting on assigned indicators and completing action plan for those not meeting set standards.
• Collaborates with outside facilities and/or agencies regarding patient's nutritional health.
• Maintains the confidentiality of all Practice information regarding patients, coworkers, and physicians.
• Performs all other relevant duties which may be contribute to the patient's wellbeing as they relate to nutrition and diet, as well as those assigned by the Support Services Manager

Required Knowledge, Skills and Abilities:

• Must possess ability to exercise expert judgment and develop insight into delivering appropriate care.
• Must possess ability to communicate effectively and express ideas clearly and concisely as this position requires frequent contact with patients, physicians, and nursing staff.
• Must possess ability to meet physical demands of the position. These include, but are not limited to, constant sitting, standing, bending, lifting, and moving. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
• Ability to excel in stressful, fast-paced work environment subject to frequent interruptions and hostile and emotionally upset patients, family members, staff, etc. Continuously serves as a mediator between patients, family members and Practice staff.
• Proficient in MS Office; Word and Excel
• Must demonstrate the ability to use company electronic medical record and lab data program after job orientation is complete.

Education, Certification and Experience Required:

• Baccalaureate or advanced degree in Nutrition, Dietetics, or equivalent.
• Registered by the Commission on Dietetic Registration, Credentialing agency for the Academy of Nutrition and Dietetics.
• LDN (Licensed Dietitian Nutritionist) status as required by the State of Pennsylvania.
• Minimum one year experience in clinical nutrition.

Physical and Environmental Conditions:

• This position requires travel between assigned facilities.
• The Renal Dietitian must expect to be exposed to the possibility of injury when using or lifting equipment and supplies, infection from undiagnosed contagious patients, disagreeable odors, and exposure to blood borne pathogens.
• The Renal Dietitian is expected to exercise "universal precautions" in the performance of all duties as this position may be subject to exposure to AIDS and Hepatitis B and C viruses.
