Job Location and Compensation 120 Corporate Woods, Rochester, New York 14623, United States of America. Full‑time, 40 hours per week, Regular Time, Worker Subtype: Regular. Compensation range: $126,256.00 – $170,456.00. Responsibilities Provide comprehensive clinical pharmacy services related to the practice area to optimize safe, effective, and cost‑effective drug therapy. Participate as a member of an inter‑disciplinary patient‑care team, offering pharmacotherapeutic and pharmacokinetic consults, patient and provider education, and development of best‑practice guidelines and policy related to pharmacotherapy. Serve as a department leader in the specialty area of practice, organize relevant departmental initiatives, and provide education and teaching for pharmacy colleagues, providers, and trainees at all levels of practice. Contribute to the overall objectives of the department as an integrated member of the pharmacy practice team. Other duties as assigned. Essential Functions Provide clinical pharmacy services for a specialized area of practice, including participating on inter‑disciplinary patient‑care team rounds as appropriate to the specialty. Offer consultative input on safe and effective drug therapy, and manage established clinical pharmacy programs or services. Monitor assigned patient populations for clinical progress—renal/hepatic function, culture results, and other applicable laboratory tests—and recommend adjustments to therapy as needed. Provide patient education and discharge counseling, authorize and verify orders, and supervise drug distribution when necessary. Document clinical pharmacy activities appropriately in the medical record or other systems. Collaborate with other clinicians and health‑care providers to implement and maintain innovative disease or medication‑management programs and clinical pharmacy services. Participate in the development of global medication‑use‑management programs within the university, including clinical guidelines, critical pathways, disease‑management, and drug‑use programs. Provide clinical leadership in the specialty by serving as a mentor, coach, and role model for pharmacists in the department. Recognize the need for mentoring and education, and reach out positively and constructively to facilitate learning and professional growth. Direct and supervise the work of technicians to ensure appropriate workflow. Conduct QA, MUE, or other research to document the value and contribution of clinical pharmacy services within the specialization. Participate in departmental Q/A processes such as reporting and following up on medication incidents, reconciling MARs, and cost‑savings initiatives. Maintain a personal and professional commitment to training future pharmacists, serving as a preceptor for pharmacy residency programs and contributing to student training experiences for IPPE and APPE rotations, including didactic lectures, case studies, and interactive teaching programs. Provide didactic lectures to divisions and departments of the medical center as requested, including nursing, medical/surgical residents, and allied‑health students. Resolve problems with appropriate and direct communication to all pertinent staff. Identify patient‑care problems and attempt issues resolution before passing to the next shift, communicating progress. Seek supervisor assistance when solutions are beyond expertise or authority. Hold active membership and participate in local, regional, and national professional organizations in clinical pharmacy and specialty areas of practice. Conduct presentations of clinical education or research results at local, regional, and national meetings. Provide continuing professional education to peers in the community, region, and nationally. Recognize expertise in specialty areas at local and national levels. Serve on departmental, hospital, or medical‑center committees or task forces. Minimum Education & Experience Doctor of Pharmacy degree required. Specialty residency training preferred. Equivalent combination of education and experience acceptable. New resident graduates or clinical specialists in their first 3–5 years of practice preferred. Licenses & Certifications New York State Pharmacy License upon hire required. Board certification in the most relevant specialty area within 2 years required.
