Telemetry RN – Unit Summary

Department: Telemetry | RN
Unit: Tele
Beds: 27 beds (expanding by 10 beds in June)
Ratios: 1 RN : 4 patients
Shift Times:

• Days: 0700–1930
• Nights: 1900–0730

Required Certifications

• BLS
• ACLS
• NIHSS

Required Experience

• Minimum 2 years Telemetry RN experience
• Must have experience with:
  • Cardiac drips
  • NIH stroke assessments
  • Post-cath recovery
  • Chest tubes

Common Diagnoses

• CHF
• NSTEMI/STEMI
• Stroke
• Sepsis
• Diabetes
• CAD
• Renal failure
• Respiratory failure
• Pneumothorax
• Post-cath lab patients

Cardiac Monitoring / Drips

• RNs do not titrate drips
• Common drips:
  • Diltiazem (set rate)
  • Amiodarone (set rate)
• No:
  • Epinephrine drips
  • Nitro drips

Cath Lab Recovery Skills

• Sheath removal: Not required
• Preferred experience:
  • TR bands
  • Fem stops

Monitoring

• Tele techs currently on the unit; transitioning to centralized monitoring
• RN responsibilities:
  • Read/interprets telemetry
  • Documents every 6 hours
• Current system: SpaceLabs
• Transitioning to Philips

Staffing Support

• Charge RN: Yes (does not take patients)
• House Supervisor: Yes
• Nurse Aides: Yes
  • Ratio: approximately 1 aide : 8–9 patients
• RT manages:
  • CPAP
  • BiPAP
  • High flow oxygen

Scheduling Requirements

• Weekend requirement: Yes
• Holiday requirement: Yes
• Call/standby: No
• Block scheduling: Not offered
• Trades with peers: Allowed
• Travelers placed based on facility needs, with requests considered when possible

Floating

May float to:

• Similar acuity units
• Med/Surg
• Boise Medical Center (like units/Med-Surg)
• Nampa Medical Center (like units/Med-Surg)
• ICU if experienced
