Description

• Conduct evaluations for patients in both inpatient and outpatient environments, tailoring unique treatment plans to meet individual needs
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functionality
• Work closely with a multidisciplinary team including nurses, physicians, case managers, and rehabilitation specialists to maximize patient outcomes
• Guide patients and their families through therapeutic goals, discharge preparation, and prescribed home-based exercise programs
• Clearly document patient evaluations, treatments, and progress in adherence with facility guidelines and regulatory standards
• Engage in care team discussions and interdisciplinary meetings to ensure cohesive patient care

Education

• A Doctorate in Physical Therapy (DPT) or a related accredited degree is necessary

Certifications

• Active state licensure as a Physical Therapist
• CPR certification is desired
• Advanced certifications in areas like neurological or geriatric therapy are advantageous but not essential
