Here’s a short video about working for Hospice of Southern Illinois: Hospice Admission Position Summary Position Summary : As a member of the dedicated admission team, the Admission Nurse has knowledge and competency related to admission compliance requirements and the hospice admission process. Works closely with the attending physician, hospice medical director, patient, and family on the initial assessment, care planning and care implementation for the patient. When a patient and their loved ones are considering hospice care, Admission Nurses help them throughout the admission process, educating on the hospice care philosophy and explaining the services our company can provide to improve the end‑of‑life care experience. Admission nurses complete an average of 1 to 2 admissions a day. Pay : $39.36 hourly. Full‑time non‑exempt position, 40 hours weekly / 80 hours bi‑weekly. Schedule : Monday‑Friday 8:30 AM‑5:00 PM (working 12:00 PM‑8:30 PM is also an option). Additional rotational on‑call schedule includes some evenings, weekends, and holidays. Weekend on‑call is limited. Location : Based out of our Belleville, IL Patient Services Office. The nurse visits patients and travels within the Belleville office’s service territory which includes St. Clair, Monroe, Randolph, Washington, Madison, Clinton, Bond, Montgomery, Macoupin, Jersey, and Calhoun. Valid driver’s license, current automobile insurance, and reliable transportation are required. The company reimburses $0.70 per mile for driving personal vehicles. Experience One (1) year hospice experience preferred. Other Qualifications Current Illinois nursing license. Certification in hospice and palliative care preferred. CPR certification preferred. Excellent leadership, managerial ability, clinical assessment, organizational and interpersonal relationship skills. Working knowledge of Medicare/Medicaid, hospice regulations, and third‑party payer expectations. Reliable means of transportation. Physical Requirements Performs repetitive tasks. Physical tasks including bathing, transferring, lifting, and turning patients in the home or home‑like setting. Prolonged periods of sitting at times. Exerts up to 100 lb of force occasionally, up to 50 lb frequently, and up to 20 lb constantly. Requires physical flexibility of bending at the knee and squat‑bend. Must manage stress appropriately, make decisions under pressure, handle multiple priorities, work with an interdisciplinary team, and manage anger/fear/hostility/violence of others appropriately. Requires clarity of vision >20 inches and Working Conditions The Admission Nurse spends approximately 50 % of time in an air‑conditioned environment with varying exposure to excessive humidity and noise. The remaining 50 % of time is spent in an automobile or patient home, which may or may not have heat/air conditioning. The position has potential for exposure to malodorous odors, infectious body fluids from patients, animal pet hair, allergens in the home environment, some exposure to noxious smells from cleansing agents, and exposure to communicable diseases and traffic. Essential Functions of the Field Staff RN/Case Manager Position Coordinates intake including collection of information, completion of referral form, obtaining attending physician certification, and computer documentation of the preadmission process, in a timely manner. Provides education on hospice care and services to all clients and referral sources. Displays knowledge of Medicare Hospice Eligibility Guidelines. Reviews the medical record and collaborates with the attending physician and hospice medical director on determining eligibility for hospice care. Explains and reviews the philosophy of hospice, hospice services, and the admission consents with the patient/family in a language and manner that the patient/family understands. Demonstrates basic registered nurse clinical skills. Gives a practical exhibition and explanation at the required annual skills day. Completes a comprehensive physical assessment. Communicates findings to the hospice medical director and utilizes information on the formulation of the plan of care necessary to treat the patient/family immediate care needs. Identifies the physical, psychosocial, emotional, and spiritual needs related to the terminal illness according to Medicare conditions of participation. Communicates this information appropriately through documentation and/or collaboration with Field Staff RN/Case Manager, Social Worker and/or Chaplain. Reviews medication, durable medical equipment (DME), and care needs with patient/family to ensure the patient/family has thorough understanding. Knowledgeable in patterns of disease progression at end‑of‑life and provides education to patient/family and facility staff at the time of admission. Uses knowledge in principles of end‑of‑life pain and symptom management. Collaborates with patient and family’s stated values and goals when considering treatment options. Includes CMS Hospice Item Set (HIS) and/or Hospice Outcomes and Patient Evaluation (HOPE) Quality Measures in discussion with patient/family and timely documents this to ensure compliance. Documents the elements of the comprehensive admission assessment in a systematic and timely manner according to Policy and Standards of Performance. Communicates admission information to the interdisciplinary team to ensure prompt start and coordination of care. Willing to assist with routine patient visits when needed. Works with the interdisciplinary team regarding the Service Intensity Add‑On and visits current patients to ensure compliance. Willing to participate in on‑call after‑hours care and picks up additional on‑call shifts when the need arises. Teamwork—demonstrates the ability to work cooperatively with others in a team environment to ensure deadlines, goals, and objectives are met. Consistently collaborates with nursing and hospice aide team to assure patient care needs are met. Identifies issues noted by the patient/family and relates them to the patient’s care team promptly. Builds relationships and collaborates effectively with internal and external customers while cultivating referrals and referral sources. Identifies and analyzes problems in the referral/admission process and offers solutions to streamline for better customer service and satisfaction. Participates in QAPI activities. Knowledgeable about performance improvement measures and works to maintain compliance. Organized and works efficiently with minimal supervision. Is a self‑starter who demonstrates flexibility, motivation, initiative, and commitment. Ensures required assigned education is finished in the specified time. Has knowledge of community resources and provides information to referral sources to provide maximum services to patients/families. Demonstrates technical knowledge and competency in hospice nursing care and communicates that expertise in conversations with patients/families/referral sources.
