HEALTH PLANNER II

Main Purpose of Job The main purpose of this position is to support, coordinate and strengthen health planning activities for the Garrett County Health Department. This position will assist with agency-wide planning, accreditation, performance management, quality improvement, grant development, administrative coordination and documentation activities that advance the department's mission and improve community health outcomes. This position will support Public Health Accreditation Board (PHAB) and Commission on Accreditation of Rehabilitation Facilities (CARF) accreditation efforts. This position will assist with the development, implementation and evaluation of the Garrett County Community Health Assessment and Community Health Improvement Plan. This position will coordinate agency performance management and quality improvement initiatives. This position will collect, analyze and report health-related data. This position will prepare planning documents, reports, presentations and meeting minutes and assist with grant writing, monitoring and reporting. This position will also support administrative processes as needed, including coordinating contracts, maintaining required documentation, tracking deliverables, organizing records and ensuring timely follow-up on planning, accreditation, grant and quality improvement activities. This position will work collaboratively with Health Department leadership, staff, community partners and stakeholders to identify health priorities, track progress toward strategic goals, promote data-informed decision-making and support efficient agency operations across programs and services.

LOCATION OF POSITION MDH, Garrett County Health Department Oakland, MD 21550 Minimum Qualifications Education: Possession of a Bachelor’s degree from an accredited college or university in Health Planning, Public Health, Health Science, Hospital Administration, or Health Care Administration. Experience: One year experience in health planning. Possession of a Master’s degree from an accredited college or university in a field identified above may be substituted for one year of experience. Possession of a Doctorate degree from an accredited college or university in a field identified above may be substituted for three years of experience. Possession of a Bachelor’s degree from an accredited college or university and one year professional experience in health planning may be substituted for the required specialized Bachelor’s degree. Applicants may substitute experience in health or hospital administration for the required experience on a year for year basis, provided the applicant has at least one year of professional experience in health planning. Health Planning experience obtained in a State Health Planning and Development Agency or in a Health System Agency as designated by Public Law 93-641 may be substituted at the rate of three months for four months of the required experience. General Health Planning experience as required above is defined as experience obtained in a professional capacity in an operation established solely for the delivery/evaluation of health services (e.g. Hospitals, H.M.O.’s, Insurance Companies). Such experience must have been the primary purpose of the position during the time in question. Health Planning duties which are performed only incidentally are not considered as qualifying.
