Watch a Day in the Life of a Coordinator [Transcript begin 00:00:00,009] Wow, I can’t believe how quickly my first year as a coordinator has flown by. Looking back, it’s been both challenging and incredibly rewarding. Right from the start, I had the opportunity to meet with inspiring leaders across the organization, including a great talk with our CEO. Those first few days set the tone for my journey as I immerse myself in training to better understand my daily responsibilities and program eligibility requirements for my clients. It’s been an incredible year of learning how to make a real difference in my community. Since I started working at the Coordinating Center, I’ve never felt alone. I have an amazing mentor for my team and a supportive team manager who checks in with me regularly. 00:00:43,029 Their guidance and encouragement have played a huge role in my growth at the Center, and I know I can always count on them for support. One of the best things about this job is the work-life integration. I’m able to manage my own schedule by working from home and visiting clients in the community. While I enjoy autonomy, my top priority is always the health and safety needs of my clients. Time management has been key, balancing documentation requirements with all of the other position responsibilities. Understanding my clients’ needs and helping them achieve their goals is at the heart of my work. A vital part of my role is collaborating with my clients and their support team to develop a plan that outlines the services they need to remain healthy and safe in the community. 00:01:30,208 These plans are person-centered and built around each individual’s unique needs, strengths, goals, and preferences. On a typical day, I check in with my clients to make sure they have the resources and support they need and work with them to address any challenges that come up. Whether it’s finding new providers, accessing equipment or supplies, or scheduling transportation to medical appointments, I know I am making a difference. One of my favorite aspects of my role is conducting site visits. Whether I’m meeting a client in their home, school, nursing facility, or other community setting, it’s always meaningful to see how I can directly support them in their own environment. At the Coordinating Center, there are countless ways to connect, grow, and give back. 00:02:14,568 Whether it’s joining a committee, participating in a team-building activity, attending an event or training, or volunteering with TCC in Action, these opportunities not only make my work more meaningful, but also help build the strong team culture that I truly value. Looking back on this past year, I’ve learned so much about care coordination and advocacy. Whenever I face a challenge, I remind myself of my why. I became a coordinator to make a difference, to offer support, and make an impact in my community. I’m proud to help individuals to live with dignity, maximize their independence, and access the resources they need to live a meaningful life.
