ECM Case Manager: Reentry & Community Care
Amity Foundation
Amity Foundation in California is seeking an ECM Advocate to coordinate medical, behavioral, and social services for high-need students across Orange County and surrounding areas. The ECM Advocate will work in custodial settings and in the community to ensure continuity of care from incarceration to reintegration, travel locally, and document services in the agency's EHR.
