Case Manager (HHSS - Housing Navigation + Tenancy)

Schedule: Monday - Friday | Full-Time (40 hours/week)

POSITION OVERVIEW

We are seeking a motivated and compassionate Case Manager to join The Good Seed CDC team in Leimert Park, CA. This position supports individuals and families experiencing homelessness through a trauma-informed, client-centered, and housing-focused approach.

The Case Manager serves as the primary point of contact for clients, helping them navigate housing instability, connect to essential services, and build long-term stability. This role requires strong organization, problem-solving skills, and a commitment to empowering clients through collaborative, strengths-based support.

KEY RESPONSIBILITIES

• Conduct initial and quarterly psychosocial assessments
• Develop and implement individualized service plans and housing stability goals
• Manage a caseload of approximately 15-20 participants
• Build and maintain trusting, professional relationships with clients
• Provide housing navigation and ongoing case management support
• Connect clients to healthcare, mental health, substance use, and community resources
• Support access to employment, education, and public benefits
• Accompany clients to appointments and community resources as needed
• Utilize harm-reduction and strengths-based approaches in all interactions
• Maintain accurate documentation, case notes, and client records
• Collaborate with multidisciplinary service providers and community partners
• Perform other duties as assigned

QUALIFICATIONS

Required:

• Bachelor's degree in Social Work, Psychology, Human Services, or related field (or equivalent experience)
• Experience in case management, housing navigation, or social services
• Ability to work with individuals experiencing homelessness, mental illness, and/or substance use disorders
• Strong organizational, communication, and documentation skills
• Ability to manage a caseload and work independently in the field
• Valid driver's license and willingness to travel locally
• Ability to pass background checks

Preferred:

• Experience in homeless services, housing programs, or supportive housing
• Knowledge of trauma-informed care, harm reduction, and strength-based approaches
• Familiarity with Los Angeles County housing and social service systems
• Experience working with integrated care teams

WORK ENVIRONMENT

This position operates in community-based and field settings throughout Leimert Park and surrounding areas in Los Angeles, CA. The role requires frequent travel, client home visits, and work in dynamic environments.

Candidates must be able to remain calm and effective in challenging situations while consistently demonstrating professionalism, respect, and a commitment to client-centered care.
