Responsibilities The Physical Therapist will primarily provide outpatient rehabilitation services in an in‑home setting for the older adult population. The setting may be a patient’s private residence or a room in an independent living facility. The role includes evaluating and treating patients in the most functional environment – the patient’s home, and developing individualized therapy programs. Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements. Adhere to department and patient treatment schedules, timely clinical and billing documentation, and attend work‑related meetings. Participate in patient care and staffing conferences. Make referrals to other disciplines and agencies to address comprehensive patient needs. Consult with the rehabilitation team to select appropriate therapeutic regimens. Adapt program and/or plan of care per patient needs. Promote a safe environment and enforce elimination of fire and safety hazards. Order supplies and equipment as necessary. Maintain, inspect, and report repair needs for equipment. Prioritize patient service and ensure high‑quality service with compassion and respect for patient rights. Screen, examine, evaluate patients, including history, systems review, and appropriate tests; establish diagnosis, identify impairments, and design plans of care. Develop and implement treatment goals and methods in collaboration with patients and caregivers; document all services. Apply therapeutic techniques: therapeutic exercise, functional training, manual therapy, airway clearance, integumentary repair, modalities, health/wellness, education on chronic disease management, fall assessment, case management. Design, fabricate, fit, and train patients in adaptive devices such as orthotics and prosthetics. Select appropriate activities based on physical/mental capacity, intelligence level, and interest. Instruct patients in skills and techniques for mobility activities; evaluate progress and behavior. Perform routine re‑examinations to modify progress plans. Incorporate health and wellness programming into treatment plans. Perform home and community mobility assessments and recommend safe mobility strategies. Perform other related duties as required. Qualifications Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility. Must have a Master's degree in Physical Therapy, or a Master's in a physical therapy related field plus a Bachelor's in Physical Therapy. Must have a Bachelor's degree in Physical Therapy plus five years of progressively responsible PT experience. If the qualifying degree was earned outside the U.S., it must be evaluated as the U.S. equivalent to a degree in Physical Therapy.
