Occupational Therapist OT - Home Health (PRN) - Fort Myers

Empath Home Health , a member of Empath Health , is seeking a caring and motivated PRN Home Health Occupational Therapist (OT). The Home Health Occupational Therapist provides exceptional care to our clients in Lee County (Fort Myers) area.

What you'll Do

• Provides treatment to the patient in compliance with agency policies and procedures and federal and state regulations.
• Performs initial and on going assessments to determine level of functioning and patient’s ability to perform activities of daily living, including OASIS assessments at appropriate time points.
• Develops and revises a plan of care in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of self-care.

What You'll Need

• Bachelor Degree in Occupational Therapy.
• Florida licensure in occupational therapy.
• Minimum of one (1) year experience as a licensed Occupational Therapist.
• Home health care experience preferred.
