Registered Nurse (RN) - Care Manager

Role: Registered Nurse (RN) Care Manager responsible for coordinating patient care across various settings. Key Responsibilities Assess, plan, monitor, and evaluate patient care plans in collaboration with healthcare teams. Manage resources to ensure cost-effective transitions to appropriate care levels while maintaining quality outcomes and patient satisfaction. Monitor care processes, collect data for quality improvement, and collaborate with physicians and healthcare staff. Educate payers, providers, and the public on healthcare trends and initiatives to enhance care delivery and resource utilization. Qualifications & Skills Associate’s in Applied Science in Nursing (minimum); Bachelor’s of Science in Nursing (preferred). Minimum of two years of experience in an acute/tertiary care facility; five years preferred. Valid NYS Registered Nurse license. Work Environment & Additional Details: Roles are performed in office or hospital units with adherence to safety protocols, utilizing appropriate protective equipment.
