RN Case Manager – Home Health Okaloosa & Walton Counties, FL (Field‑Based) 13‑Week Contract | Competitive Pay | Mileage Reimbursement Why You'll Love This Role Monday–Friday schedule (8 AM–5 PM) with minimal call rotation Autonomy in the field while making a meaningful patient impact Mileage reimbursement for all travel Opportunity to work with a collaborative interdisciplinary team Competitive weekly pay (~$2,082/week local package) Position Overview We’re seeking a dedicated RN Case Manager to coordinate and manage patient care across the continuum for home health patients in Okaloosa and Walton counties. In this role, you’ll ensure high‑quality, cost‑effective care while advocating for your patients and supporting their long‑term health outcomes. Key Responsibilities Conduct comprehensive assessments of high‑risk patients Develop and implement individualized, patient‑centered care plans Collaborate with physicians, social workers, and care teams Coordinate transitions of care and community support services (housing, transportation, etc.) Monitor clinical, functional, and financial outcomes Promote preventative care and reduce hospitalizations Maintain accurate documentation using HCHB/OASIS Serve as a patient advocate ensuring privacy and compliance Requirements Active, unrestricted RN license (Florida) Home Health experience required (2+ years preferred) Case Management experience preferred Experience with HCHB and OASIS strongly required Current CPR certification (if applicable) Valid driver’s license + proof of auto insurance Ability to travel within assigned counties Assignment Details Start Date: July 6, 2026 Schedule: Monday–Friday, 8 AM–5 PM Additional: 1 weekend per month (preferred) 1 on‑call night per week Duration: 13 weeks Hours: 40/week Interview: Required (in‑person) What Makes You a Great Fit Strong clinical judgment and decision‑making skills Ability to manage complex patient cases independently Excellent communication and coordination skills Passion for patient advocacy and improving outcomes Benefits Competitive pay & weekly paychecks Health, dental, vision, and life insurance 401(k) savings plan Awards and recognition programs *Benefit eligibility is dependent on employment status.
