Highlights of the RN Case Manager Partner Position:

• Hold RN license in state of NY
• At least three years’ experience working with an adult populating providing supportive care services in an adult care or long-term care setting.
• Experience with Dementia-specific care.
• Strong assessment skills; geriatric experience (preferred)
• May Travel up to 25%

The salary range for this position is $69,377 to $110,823.
