General Information
Role Responsibilities
- Provide case management services in an acute care setting.
- Ensure effective patient care coordination and discharge planning.
- Collaborate with a multidisciplinary team to meet patient needs.
Qualifications
Must Have:
- Experience in an acute care setting.
Preferences:
- Ability to multitask and work independently.
- Positive attitude and strong team collaboration skills.
- Reliable with excellent communication and time management skills.
