In this role, you will advocate for patients, conduct holistic assessments, develop individualized care plans, and empower patients and families to successfully self-manage their care. This position plays a crucial role in supporting hospital throughput, coordinating complex and challenging discharges, and providing post-discharge care coordination to promote safe and successful transitions.A caregiver in this role works days from 8:30 a.m. – 4:30 p.m. or 9:00 a.m. – 5:00 p.m.A caregiver who excels in this role will:Ensure compliance with standards of the accrediting bodies, state mandated revised code and the CMS Conditions of Participation.Promote interprofessional synchronization throughout the acute episode.Complete effective handoffs to the next level of care or community to ensure continuity of care.Proactively assess, identify and work to mitigate the risk posed by barriers to patients' transitions of care.Work with the patient and/or care partner(s) to develop a comprehensive, individualized transitional care plan.Participate and lead in patient rounding activities, huddles and committee meetings.Minimum qualifications for the ideal future caregiver include:Graduate from an accredited school of Professional NursingCurrent state licensure as a Registered Nurse (RN)Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red CrossThree years of recent RN clinical experiencePreferred qualifications for the ideal future caregiver include:Bachelor's of Science in Nursing (BSN)Certification by an approved entity within one year of eligibility (CTM, ACM, CCM or AHA)Two years of recent experience in utilization review, quality or care managementKnowledge of acute care regulatory/accreditation requirementsDischarge planning and/or Quality Improvement experiencePhysical Requirements:Requires normal or corrected hearing and vision to normal range.Ability to perform work in a stationary position for extended periods.Ability to communicate and exchange accurate information.Ability to work with physical records, such as retrieving and filing.Ability to operate a computer and other office equipment.Ability to travel throughout the hospital system.In some locations ability to lift up to 10 pounds.Personal Protective Equipment:Follows Standard Precautions using personal protective equipment as required for procedures.The policy of Cleveland Clinic Health System and its system hospitals (Cleveland Clinic Health System) is to provide equal opportunity to all of our caregivers and applicants for employment in our drug free environment.
