Arc Hospice & Palliative Care is seeking a Hospice RN Case Manager to deliver skilled, personalized nursing care aligned with each patient's goals and needs.About the RoleThe Hospice RN Case Manager is responsible for comprehensive nursing assessment, care planning, and ongoing clinical oversight for hospice patients and their families. This role ensures high-quality, compassionate care while coordinating closely with the interdisciplinary team.Key ResponsibilitiesComplete initial and ongoing assessments of patients' physical, emotional, psychosocial, and environmental needsCoordinate with physicians for certification and recertification of terminal illness per Medicare regulationsServe as the clinical point person for assigned patients, including symptom escalation, advanced care planning, and advocacyCollaborate with all providers and support systems to ensure seamless, compassionate hospice careDevelop and update individualized care plans with patients, families, and the IDGProvide skilled nursing care and document interventions and outcomes accurately and promptlyEnsure documentation meets regulatory and billing compliance standardsEducate patients and families on disease progression, symptom management, medications, and hospice philosophySupervise hospice aides and revise aide care plans as neededParticipate in IDG meetings with physicians, social workers, chaplains, and bereavement staffCommunicate effectively with the interdisciplinary team and clinical leadershipParticipate in the on-call rotation and respond to urgent needsEscalate care needs to the medical director or covering provider when appropriatePerform other duties as assignedQualificationsGraduate of an accredited nursing program; Associate degree or higher preferredCurrent RN license in the state of practice or valid multistate compact licenseMinimum one (1) year of hospice, palliative care, or home health experience preferredCPR certification and valid driver's license requiredStrong clinical judgment, documentation accuracy, and communication skillsKnowledge of hospice regulations and Medicare Conditions of Participation preferredAbility to work independently while collaborating across interdisciplinary teamsExcellent communication, critical thinking, and organizational skillsCommitment to Arc Hospice & Palliative Care's mission and valuesThe compensation range for this position is shown below and reflects the expected pay range for the role. Arc Hospice & Palliative Care is committed to transparency and equity in all compensation practices.Final pay is determined based on factors such as experience, licensure, location, and business needs at the time of hire.
