About the Position

Home Health RN Profession: Nursing Specialty: Home Health RN Duration: 13 weeks Shift: Monday - Friday Hours per Shift: 8am to 4pm Experience: Minimum 2 years License: Active nursing license required Certifications: OASIS experience required Must-Have: - OASIS experience - Knowledge of evidence-based practice - Ability to collaborate with interdisciplinary teams Description: Utilizing the nursing process, assess patient and caregiver needs to provide quality care. Develop and communicate a realistic plan of care for each patient. Include patients and caregivers in the care plan process. Serve as Case Manager for assigned patients and actively manage case loads. Effectively complete OASIS documentation in accordance with agency policies and regulations. Implement medical plans of care and nursing interventions as necessary for safe and effective patient care. Stay updated on high-tech skills, including IV use and maintenance, wound VAC, and complex wound care. Work collaboratively with LPNs and ensure that the plan of care is followed. Evaluate aspects of the implemented plan of care to meet patient and caregiver needs. Maintain accurate documentation of nursing care provided to ensure effective information integration. Follow agency protocol with documentation processes. Perform patient and caregiver education to ensure informed decision-making. Identify interdisciplinary needs of patients and communicate documented information in a timely manner. Accurately fill out various agency forms in a timely manner. Carry out visits efficiently and communicate needed information to scheduling staff. Assess patient needs for HCA services and provide ongoing supervision accordingly. Transmit patient incident reports to quality improvement specialists as necessary. Coordinate safe patient discharges and ensure all paperwork is processed. Support the plan of care by coordinating with physicians, providers, and team members. Participate in monthly peer review audits and accept feedback professionally. Schedule and facilitate case conferences to enhance communication with team members. Advocate for patient care issues to ensure quality and type of care meets patient/family needs. Conduct comprehensive assessments of patient and caregiver needs. Participate in interdisciplinary team meetings to develop individualized care plans within required timeframes. Stay informed on pain control, symptom management, and high-tech skills. Perform patient and caregiver education on various health-related topics. Coordinate care with various healthcare professionals to ensure effective patient and family support.
