For more than 55 years, Family Health Centers of San Diego (FHCSD) has provided caring, affordable, high‑quality health care to the uninsured, low‑income, and medically underserved. FHCSD operates over 90 sites across San Diego County, serving over 227,000 patients annually, 91% of whom are low‑income and 29% uninsured. Our services range from primary care and behavioral health to specialty clinics, including cardiology, podiatry, endocrinology, and more. Responsibilities Respond to routine messages in the electronic health record within 2 days and urgent messages within 1 day. Attend nursing in‑services and complete required assignments within 2 weeks. Build positive relationships, inspire high performance, prepare career‑development plans, and manage accountability. Coach, counsel staff, and administer timely disciplinary action. Communicate effectively with interdisciplinary care teams, including physicians, PSRs, medical assistants, and others. Attend department meetings and maintain accurate, timely documentation per department standards and insurance requirements, including patient history. Follow policies and procedures to ensure positive patient outcomes. Ensure medical assistants document vital signs and pre‑assessment questions. Maintain clinic workflow, assign lunch times and breaks, set up exam rooms, order supplies and vaccines, support provider productivity, review patient problem lists before visits, and call refills to pharmacy. Maintain compliance with TJC, DHS, CHDP, and Medi‑Cal Managed Care requirements. Participate in continuous‑improvement activities and implement Lean tools such as 5S and A3. Perform nursing tasks, demonstrating clinical competency: measuring height, weight, vital signs; hearing and vision screenings; administering IM, PO, subcutaneous, inhalation medications; PPD testing and interpretation; performing EKGs; ear lavage; setting up sterile fields; disinfection; oxygen tubing; suctioning; straight‑cath; starting IVs; completing standing orders; and performing wet or dry testing and phlebotomy when trained. Ensure staff receive all new‑hire and annual trainings on time. Requirements At least 3 years of experience in an ambulatory or similar care setting. California State Board LVN license required. Current CPR for Healthcare Providers certification required. Graduation from a recognized practical nurse program or equivalent. High school graduate or GED equivalent. Bilingual in Spanish/English strongly preferred; may be required based on assignment. Travel between sites occasionally required. Valid California driver’s license, automobile, and proof of minimum car insurance as required by California law, with an acceptable driving record. Reasonable accommodation may be provided on a case‑by‑case basis. Compensation and Benefits Regular full‑time, Monday‑Friday 8:30a.m.–5:30p.m. on‑site. Salary range: $36.00–$42.99 per hour.
