Travel RN Care Manager: LTSS & Waiver Services
Molina Healthcare
Molina Healthcare is seeking an experienced RN Care Manager to support long-term services and supports (LTSS) for high-need members. You will coordinate care with a multidisciplinary team and focus on achieving positive health outcomes while managing costs. You will assess needs, develop and implement care plans, monitor progress, and promote integration of behavioral health and LTSS services.
