Optimal Care
Optimal Care is seeking a Hospice RN Case Manager based in Michigan. You will provide skilled nursing care to patients in their homes, coordinating with physicians, facilities, and an interdisciplinary team to ensure comfort, dignity, and quality of life. You will build meaningful relationships with patients and families while managing a dynamic caseload across Wayne County and surrounding areas.
