Role Summary
We are seeking a compassionate and experienced Home Health Registered Nurse (RN) to provide high-quality patient care in the home setting. The ideal candidate will have hospice experience and a strong commitment to supporting patients and families through individualized care plans.

Requirements

Key Responsibilities

•  

Provide skilled nursing care to patients in their homes

•  

Perform patient assessments and develop individualized care plans

•  

Administer medications, treatments, and wound care as prescribed

•  

Monitor patient progress and communicate changes to the healthcare team

•  

Educate patients and families on disease management, medications, and care plans

•  

Maintain accurate and timely documentation

•  

Coordinate care with physicians, case managers, and interdisciplinary teams

•  

Ensure compliance with home health and regulatory standards

Required Certifications

•  

BLS (Basic Life Support) Certification

Qualifications

•  

Active RN License

•  

Minimum 1 year of hospice experience required

•  

Home Health experience preferred

•  

Strong assessment, communication, and patient education skills

•  

Ability to work independently and manage a caseload effectively

•  

Valid driver's license and reliable transportation preferred
