CHWs play a vital role in strengthening community wellbeing, promoting health equity, and connecting residents to the services and supports that help them thrive. Grounded in culturally responsive public health practice, the CHW position blends field-based outreach, client navigation, and health education with strong documentation and collaborative teamwork. While many activities take place directly with community members, the role also requires substantial behind the scenes preparation, resource coordination, and reliable follow-through to ensure smooth service delivery. Working closely with community partners, internal teams, and residents across diverse settings, CHWs help identify needs, reduce barriers, and amplify community voices—bringing curiosity, compassion, professionalism, and respect to every interaction. Key responsibilities include, but are not limited to:Maintain a welcoming, dependable, and professional presence at community events, health fairs, and outreach activities.Build trust with residents while providing culturally appropriate health education tailored to audience needs and literacy levels.Support individuals and families by navigating health, social, and community systems; making appropriate referrals; and following up on identified needs.Document interactions, referrals, follow ups, and case notes accurately and on time—helping meet program, grant, and reporting requirements.Share insights on community challenges, patterns, or emerging needs to strengthen workflows, resource pathways, and service delivery.Represent the program with cultural humility and professionalism, contributing to special projects, health promotion events, and emergency response activities.Collaborate with team members, partners, and community agencies to coordinate services and support outreach efforts.Hiring salary is commensurate with experience.Bachelor's degree or equivalent and one (1) year of experience working directly with the public in community outreach, education, customer service, social services, human services, healthcare, public health, workforce development, nonprofit programs, case coordination, advocacy, or related fields; or an equivalent combination of education and experience. Preferred Qualifications:Knowledge of public health concepts, disease prevention, health equity, and the social determinants of health.Skill in building trust with community members and delivering clear, culturally responsive health education.Ability to manage moderate to complex client needs requiring multi step navigation and coordination across systems.Strong documentation skills and comfort with Microsoft 365 applications.Experience participating in outreach events, community engagement activities, and collaborative public facing work.Bilingual proficiency in a language commonly spoken within the community (Spanish preferred).Job Contingencies and Special Requirements:Valid driver's license and good driving record (records may be reviewed annually).Must pass pre-employment background checks, including fingerprinting and DMV review.Completion of onboarding health screenings, including TB screening and immunizations required for healthcare workers.Equal Employment Opportunity Loudoun County Government prides itself on fostering a diverse and inclusive workplace that promotes employee wellness and work-life balance.
