Assistant – Case Management The Assistant – Case Management supports the case management leadership team in utilization review, discharge planning, and related tasks to ensure smooth operation of case management functions and high‑quality care. Responsibilities Assists case management leadership with day‑to‑day administrative tasks to support utilization review and discharge planning. Communicates effectively with internal teams and external stakeholders to address case management needs. Coordinates with external providers and resources to support utilization review and discharge planning efforts. Prepares and maintains reports, data, and metrics related to case management activities. Follows up on pending tasks, updates faxes, obtains required documentation, and ensures compliance with policies and procedures. Monitors and tracks case status, ensuring timely updates in case management processes. Identifies and resolves gaps in processes or documentation to enhance efficiency and effectiveness. Performs additional duties as assigned to support the goals and objectives of the department. Collaborates with case managers, healthcare providers, and team members to facilitate smooth transitions of care. Additional Qualifications Able to prioritize and manage multiple tasks and deadlines, demonstrate critical thinking and problem‑solving skills, manage sensitive and confidential patient information, exhibit strong interpersonal and communication skills, adapt to changing priorities, and maintain accuracy in documentation.
