Case Management Assistant

Case Management Assistant The Case Management Assistant (CMA) will work under the direction of the RN Case Manager or Social Worker to assist with facilitating patient discharges requiring post-acute placement or services, and will identify and report barriers to discharges. The CMA will coordinate timely referrals and facilitate payor authorizations where applicable, and also provide support in areas that do not require licensed personnel. Responsibilities Coordinate with RN Case Manager and Social Worker for support needed and communicate updates on progress throughout the day. Provide clerical support of multidisciplinary rounds, including updating NATE Tempo at direction of team. Make referrals through the web-based referral system (nH Discharge/Other) to the specific post-acute care providers requested, sending the appropriate packet of information from the medical record. Schedule appointments for outpatient hemodialysis. Monitor all patients with PAC referrals and intervene to expedite placement. Prioritize workload to ensure timely contact and intervention with PAC settings. SEND additional information to PAC provider when requested. Communicate with RN Case Managers/Social Workers regarding the status of PAC referrals, bed availability, and other barriers that cannot be resolved. Communicate results of PAC referrals with the patient, including having the Patient Choice letter signed. Monitor Medicare patient’s planned discharge dates and deliver the Important Medicare Message (IMM) to the patient per facility policy. Deliver the Beneficiary Notice to patients identified as being in the BPCI-A program. Obtain physician signature on required case management documents. Set up transportation/send information to Transfer Center and generate discharge packets for PAC transfers based on facility practice. Utilize Case Management systems, standards, and processes. Maintain knowledge of and actively develop relationships with PAC providers and payers. Demonstrate knowledge of regulatory requirements, HCA Ethics and Compliance policies. Practice and adhere to the "Code of Conduct" and "Mission and Value Statement." Qualifications Associate Degree preferred. Medical knowledge and terminology required. Minimum of two years experience in a health care setting required. Currently licensed/certified as a health care professional (CNA, EMT) preferred.
