Case Management Assistant responsibilities Assists with completing tasks necessary to facilitate patient and family access to agencies, transportation, and services within and beyond acute care Responsible for assisting with distributing regulatory documents to identified patients Educating patients about the appeal process related to their discharge Performing multiple activities that support a seamless transition of patients from the inpatient hospital setting, using current knowledge of hospital processes and community service Requires frequent contact with patients and patient representatives including but not limited to hospital staff, community resource agencies and payor representatives Location and Working Hours Location: Dunedin, FL Status: PRN As needed Schedule: 8:00 AM – 4:30 PM On Call: No Benefits Paid time off 401k match and additional yearly contribution Yearly performance appraisals and team award bonus Community discounts and more Education Required High School or Equivalent Preferred Associate's Experience Required 1 year in Medical Related Field Equal Opportunity Employer Veterans/Disabled #J-18808-Ljbffr Baycare
