Therapist - Physical Therapy Assistant

Job Description Job Summary: The Physical Therapist Assist assists with patient care plans under the supervision of licensed physical therapists. This role supports therapeutic interventions to help patients improve mobility, function, and independence. Responsibilities: Assist in implementing individualized physical therapy treatment plans as directed by the supervising PT. Support therapeutic exercises, mobility training, and gait/balance activities for assigned patients. Document patient care activities and progress in EPIC, ensuring accuracy and timeliness. Maintain safe treatment environments and follow infection control and safety procedures. Communicate patient responses and clinical observations to the care team to support plan-of-care updates. Assist with patient education focused on home exercises and self-care strategies. Qualifications: Hold an active MN Physical Therapy Assistant license or demonstrate proof of application/ability to obtain licensure promptly. Possess at least 1 year of relevant experience as a Physical Therapy Assistant. Demonstrate familiarity with documentation workflows in EPIC (preferred). Maintain current BLS certification. Meet travel compliance requirements, including eligibility criteria for non-local status. Complete required onboarding steps promptly, including MN-specific fingerprinting/livescan where applicable.
