Roles & Responsibilities:

• Provide patient care and support to residents in a skilled nursing facility.
• Monitor and document patient conditions and responses to treatment.
• Administer medications and treatments as prescribed by healthcare providers.
• Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
• Educate patients and families on care plans and health management.
• Assist with daily living activities and promote patient dignity and independence.
