Centra’s PACE (Program of All-Inclusive Care for the Elderly) is a nationally recognized model of care serving adults aged 55 and older in our community. The mission of PACE is to deliver person-centered care in the setting that’s most appropriate for each participant—whether that’s in their own home, with family, in long‑term care, or assisted living. CMG also hosts the Lynchburg Family Medicine Residency, a training program for future family physicians. Responsibilities Monday – Friday, 8:00 am – 5:00 pm; no weekends or holidays. Work with a dedicated interdisciplinary team—including providers, support staff, physical therapists, occupational therapists, dietitians, and social workers. See 5 patients per day—small volume. Call is once every 7 weeks, 5 pm – 8 am; triaged before reaching you, no out-of‑home calls. Build meaningful relationships with our elderly population. Required Qualifications Experienced providers in palliative, hospice, primary care, or long‑term care are encouraged to apply. Qualifications Doctoral degree from an accredited Physician Assistant School.
