$45–$50/hour | Full-Time | Benefits Ultimate Therapy is hiring a Physical Therapist (PT) – Full-Time Autumn Lake at Norwalk | Norwalk, CT Ultimate Therapy is hiring a Physical Therapist (PT) to provide rehabilitation services at Autumn Lake at Norwalk. Join a supportive clinical team focused on helping residents restore movement, increase strength, and make meaningful progress toward their functional goals. What You'll Do Perform comprehensive physical therapy evaluations and establish individualized plans of care Provide skilled treatment addressing mobility, strength, balance, gait, transfers, and endurance Establish functional goals and track resident progress throughout the rehabilitation process Modify treatment plans based on clinical findings and response to therapy Educate residents and caregivers on exercises, mobility techniques, and fall-prevention strategies Complete required clinical documentation accurately and on time Coordinate care with PTAs, OTs, SLPs, nursing staff, and other healthcare professionals What We Offer $45–$50/hour Full-time schedule Medical, dental, and vision coverage 401(k) retirement plan Paid time off and holiday pay Continuing education opportunities Supportive rehabilitation leadershipQualifications Active Connecticut Physical Therapist license required Graduate of an accredited Physical Therapy program Strong clinical judgment and interpersonal skills Post-acute or skilled nursing experience is a plus, but not required New graduate Physical Therapists are welcome to apply If you're a PT looking for a collaborative environment where you can make a direct impact on residents' mobility and recovery, we encourage you to apply.
