About the Role
Corus Home Health in Albuquerque is seeking a compassionate Physical Therapist to join our dedicated home health team. This is your chance to make a lasting impact through one-on-one, in-home care that promotes strength, mobility, and independence. Key Responsibilities
- Evaluate patients and create personalized therapy plans in the home setting
- Deliver evidence-based interventions to restore mobility and function
- Educate patients and families on exercises and safety strategies
- Collaborate with the interdisciplinary team to ensure coordinated, goal-oriented care
- Maintain accurate and timely documentation to meet quality and compliance standards
- Adapt care plans based on progress and physician feedback
- Support patients through discharge planning and wellness strategies
What We’re Looking For
- Licensed Physical Therapist (PT) in the state of New Mexico
- At least 1 year of clinical experience (home health preferred)
- Skilled in patient communication, documentation, and problem-solving
- Self-motivated and comfortable working independently
- Proficient with electronic health records and mobile charting
- Passionate about function-focused, relationship-driven care
