The Position:
The Part-time ElderCare Registered Nurse (RN) provides person-centered chronic care management services to seniors through a hybrid care model incorporating telephonic, virtual, and in-person field-based visits. This role focuses on chronic disease management, care coordination, patient education, and health promotion to support quality of life and reduce avoidable hospitalizations.
Working as part of a multidisciplinary care team, the RN collaborates with providers, caregivers, and community resources to develop and manage individualized care plans, support care transitions, and ensure continuity of care for program participants and their families.
This role includes both telephonic health management and community-based patient visits, requiring flexibility and adaptability to meet patient and program needs.

Market-Competitive Pay: $48.00 – $61.00 per hour
Schedule:
- Part time, approximately up to 24 hours per week
- Hybrid role: Approximately 50% telephonic/virtual care and 50% field-based visits
- Schedule varies based on patient census and program needs
Responsibilities:
Clinical Care
- Conduct comprehensive patient assessments via telephone, virtual visits, and in-home/community visits
- Develop, implement, and monitor individualized care plans for patients with multiple chronic conditions
- Provide disease-specific education and self-management support
- Deliver telephonic health coaching and chronic care management services
- Monitor patient status and identify changes requiring provider intervention
- Document accurately, timely, and thoroughly in the electronic medical record
- Participate in interdisciplinary team meetings and care planning discussions
Care Coordination
- Coordinate care across providers, caregivers, and community resources
- Support transitions of care following hospitalizations or emergency visits
- Serve as a consistent point of contact for patients and families
- Assist with advance care planning conversations as appropriate
- Facilitate connections to community-based services during field visits
Program Support and Quality
- Ensure compliance with CMS requirements for Chronic Care Management (CCM) and related programs
- Support quality improvement initiatives and workflow refinement
- Maintain working knowledge of chronic care management best practices
Regulatory Compliance and Quality Assurance
- Ensure compliance with organizational policies, HIPAA, and applicable regulations
- Maintain current knowledge of chronic disease management and geriatric care trends
- Support accurate documentation for billing and quality reporting
Requirements:
- Graduate of an accredited School of Nursing
- Current and active California RN license in good standing
- Minimum 3–5 years of nursing experience in chronic disease management, geriatrics, home health, hospice, or primary care
- Strong clinical assessment and care coordination skills
- Experience providing telephonic care management and/or field-based patient care preferred
- Proficiency with EMRs, telehealth platforms, and digital documentation
- Excellent communication, organization, and time management skills
- Ability to work independently in both remote and field environments while collaborating with an interdisciplinary team
Preferred Qualifications:
- Bachelor’s degree in nursing (BSN) preferred
- Certification in Chronic Care Management (CCM) or related field
- Training or experience in motivational interviewing or health coaching
- Knowledge of trauma-informed care
Work Environment & Expectations:
- Hybrid role based out of Rancho Cordova
- Combination of remote work and in-field patient visits
- Travel within service area required for community-based care
- Part-time hours, flexible schedule aligned with patient and program needs
