Physical Therapist - Full Time

Minimum Requirements:
Bachelor‘s, Master‘s, or Doctoral Degree in Physical Therapy from an accredited school approved by the APTA.
Current Wisconsin Physical Therapist (PT) license required.
BLS Certification from AHA or ARC may be required.
Experience working with adult and geriatric populations, preferably in a skilled nursing facility or long-term care setting.
Clinical documentation experience. Experience with Net Health is a plus.
Strong communication and interdisciplinary teamwork skills.

Your Responsibilities
Provide compassionate and effective physical therapy services to adult and geriatric residents in a skilled nursing facility.
Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
Support residents in achieving personal goals and rehabilitation milestones.
Accurately document care and maintain compliance standards. Experience with Net Health is a plus.
Collaborate with therapy and nursing teams to ensure patient-centered outcomes.
Educate residents and families on safe techniques and strategies to maximize independence.
Participate in team meetings and contribute to care planning.
