About the Role We're looking for an experienced Advanced Practice Provider to practice at the top of their license in a supportive, evidence-based addiction medicine model. This position may be based in either of our two Nashville care settings , depending on organizational need and your experience and interest: Inpatient SUD Consults — providing an addiction medicine consult service within our hospital partner settings, serving as a primary point of contact for hospital providers and filling a critical gap in SUD care. Outpatient Clinic — managing a patient panel in our Nashville clinic, delivering longitudinal, relationship-based care alongside a full interdisciplinary team. Provide expert compassion and patient-centered advocacy through the application of Motivational Interviewing principles. Practice up-to-date, life-saving, cutting-edge SUD therapies with close guidance, oversight, and mentorship from specialty-trained addiction experts. Collaborate with interdisciplinary team members — including Medical Assistants, Peer Recovery Specialists, Therapists, and physicians — to formulate and execute patient care plans. Support continuity of care across settings and into community resources, reducing barriers to long-term recovery. Demonstrate flexibility and proactive problem-solving in adapting to diverse patient needs and situations. Continuously seek out new knowledge and stay informed on advancements in addiction medicine, maintaining a lifelong learning mindset. Provide occasional cross-coverage for colleagues in the other care setting as needed, ensuring seamless patient care and team support. If you join our inpatient consult team, you will also: Conduct inpatient consultations for patients with substance use disorders and related comorbidities. Serve as a primary point of contact for hospital providers, delivering timely consultations, treatment recommendations, and mission-driven support to patients. Work closely with attending physicians, hospitalists, nursing staff, and case managers to develop and implement evidence-based treatment plans. Ensure smooth handoffs to Steadfast Health outpatient clinics and community resources. Build strong, respectful relationships with hospital staff, contributing to an integrated, collaborative model of care within the hospital system. If you join our outpatient clinic team, you will also: Manage a full patient panel, delivering longitudinal care with minimal ramp-up time. Create a warm and welcoming clinic environment, fostering trust and strong relationships with patients and the surrounding community. Pioneer novel ways of improving access and overall care for disadvantaged and marginalized populations. Respond calmly and effectively to urgent or crisis situations in the clinic. Background NP (FNP, AGNP, PMHNP preferred) or PA-C (Internal, Family, Emergency, or Hospital Medicine experience preferred), licensed in the state of Tennessee . Minimum of 2+ years of clinical experience as a provider. Outpatient experience is preferred for our clinic-based role; hospital-based experience is preferred for our consult role. Comprehensive medical and/or psychiatric evaluation and management experience in substance use disorder treatment. Strong knowledge of SUD diagnoses, medications, and treatment modalities. Demonstrated ability to work independently, whether managing a full patient panel or navigating hospital systems, workflows, and interdisciplinary teams. Ability to effectively interact with diverse staff, patients, and community members. Excellent interpersonal skills, a flexible, can-do mentality, and a commitment to continuous learning and professional development. Please note that this schedule may evolve based on organizational needs, and flexibility may be required as Steadfast Health continues to grow. Location Nashville, TN. Our outpatient clinic is located at 1900 Patterson St, Nashville, TN 37203. Inpatient consult work takes place within our Nashville-area hospital partner settings. Compensation This is a full-time, W2 position offering a salary of $130,000–$145,000, commensurate with experience, with a 5% monthly performance-based bonus and a $2,000 sign-on bonus. Ability to communicate clearly and effectively with patients, families, and clinical staff — both verbally and in writing, in person and over the phone — including during emotionally charged or crisis situations. Capacity to occasionally lift or move items up to 25 pounds (such as medical supplies, patient records, or small equipment). Ability to respond quickly and calmly to urgent or emergency situations. Flexibility to adapt to a fast-paced, variable care environment while maintaining a patient-centered focus.
