Certified Medical Assistant – Chronic Care Management (CCM)

We are seeking a compassionate, organized, and motivated Certified Medical Assistant (CMA) to join our growing Chronic Care Management team. This role focuses on supporting patients with chronic medical conditions through ongoing communication, care coordination, education, and follow-up.

The ideal candidate is detail-oriented, comfortable communicating with patients by phone, and passionate about improving long-term patient outcomes.

Responsibilities

Conduct monthly patient outreach calls for Chronic Care Management services

Document patient interactions accurately in the electronic medical record (EMR)

Assist with care coordination, medication reconciliation, and appointment scheduling

Monitor and track patient care plans and follow-up needs

Communicate with providers regarding patient concerns or changes in condition

Support compliance with CMS CCM program requirements

Educate patients regarding chronic disease management and preventive care

Maintain confidentiality and HIPAA compliance at all times

Qualifications

Current Certified Medical Assistant (CMA) certification required

High school diploma or equivalent required

Prior experience in primary care, internal medicine, family medicine, or chronic care management preferred

Strong telephone communication and customer service skills

Experience with EMR systems preferred , eClinicalWorks a plus (ECW)

Ability to multitask and work independently

Bilingual candidates encouraged to apply

Preferred Skills

Knowledge of Medicare Chronic Care Management programs

Experience with remote patient communication and care coordination

Strong organizational and documentation skills
