RN - Case Management

RN Case ManagerLocation: Portland, MaineAssignment Duration: This is a 13-week contract position.Schedule: Monday through Friday, 8:00 AM - 4:30 PM. Every third weekend and holiday required.Job Description: The RN Case Manager will be responsible for discharge planning and care coordination within a hospital setting.
